Business Basics

Your name and business name appear on the report cover. Your email is how we deliver it. Your website URL is what we run every automated check against — 26 of the 47 checks run directly against this URL.

We send the finished report here. Double-check this.
Include https://. If you have no website, enter "none" and we will note it in the report.
Used to check local SEO signals (checks localseo-07, localseo-08).

How Customers Reach You

We check whether phone numbers on your site are tap-to-call, whether emails are mailto-linked, and whether your site has a booking path. We need your actual main number and email to verify those checks against the right values (checks lead-03, lead-04, lead-05).

The primary line customers should call. We verify it appears click-to-call on your site.
The address visitors should use to reach you. Leave blank if not publicly listed.
Calendly, Acuity, TidyCal, your CMS booking page, etc. We check whether this is discoverable from your homepage (check lead-05). Leave blank if none exists.

Google Business Profile

Three checks depend entirely on your GBP status — whether it exists and is claimed, whether the category is accurate, and whether the name/address/phone match your website (checks localseo-01, localseo-02, localseo-03). We cannot log in to verify these ourselves, so your answers here are the evidence.

Do you have a Google Business Profile that you have claimed and verified? *
Find this by searching your business on Google Maps and copying the URL. Allows us to compare your NAP directly. Leave blank if no claimed profile exists.
The category shown on your listing. Used to verify it matches your actual service (check localseo-02).

Lead Handling & Response

Five checks in the AI/Tech Stack area are about what happens after someone reaches out — how fast you respond, what you do with missed calls, whether you use a CRM, and whether you have automated reminders and review requests (checks stack-01, stack-02, stack-03, stack-04, stack-05). Your honest answers here directly determine the findings.

When a call comes in and you miss it, what happens next? *
On average, how long does it take you to respond to a web lead (a form submission or email inquiry)? *
How do you track leads and clients? *
How do you send appointment reminders to clients? *
After completing a job or service, do you have a process to request a review? *

Proposals & Quotes

Check stack-08 is about whether your proposal and quoting process is fully manual each time. This adds time to every sales cycle and is one of the most common efficiency leaks we find.

How do you produce proposals or quotes for new clients? *

Contact Form on Your Site

Check lead-02 asks whether your contact form produces a visible confirmation when submitted. We test this ourselves by submitting a form with dummy data — but only if we know the form exists and where it is. We will never submit your real booking form with test data without asking.

We will submit a clearly labeled test entry to verify the confirmation message. If you have a booking form we should not test, tell us below.

Anything Else

Optional. If there is a specific area you already suspect is broken, or context that would help us interpret a finding, note it here. It does not change how we run the 47 checks, but it may shape how we explain a finding in the report.

Helps us match this intake to your order if you purchased the Diagnostic + Strategy Call tier.

Submitting this form starts your 3-business-day delivery clock. You will receive a confirmation email at the address you provided.
