Informed Consent & Enrollment
Companion Care is a friendly voice companion. It checks in with you and can gently ask a few well-being questions over time. Before you begin, we want you to understand what it does — and give your permission. You are in control. You can say no, and you can stop at any time.
What you are agreeing to
1 · We collect some information to help you
- How you are feeling (your mood, and short answers about sleep, medicine, eating, or pain).
- Your answers to short, well-known well-being questions about loneliness, mood, and worry (called UCLA-3, PHQ-2, PHQ-9, and GAD-7).
- What you say out loud — we turn your words into text so the companion can understand you.
2 · About your voice
We use your voice only to understand your words. We do not make a "voiceprint," and we do not use your voice to recognize who you are. We delete the recording of your voice shortly after we turn it into text. We keep only the words you meant to share.
3 · These questions are a check-in, not a diagnosis
The well-being questions screen for things like loneliness, low mood, or worry. They do not diagnose any condition and are not a medical test. Only a qualified health professional can diagnose or treat you.
4 · This is wellness support — not medical care and not an emergency service
Companion Care is here for companionship and check-ins. It is not a doctor, nurse, therapist, or emergency line. It cannot call an ambulance or send help.
5 · You are talking to an AI, not a person
The companion is a computer program (artificial intelligence). It is not a live human and not a licensed clinician. It does its best to be kind and helpful, but it can be wrong. Please use your own judgment and talk to real people you trust.
6 · Telling a caregiver (only if you say yes)
If you choose, we can let one caregiver you name know when something seems off — for example, if your well-being scores change a lot. Your caregiver sees alerts and trends, not word-for-word what you said. You choose whether to turn this on, and you can turn it off any time.
7 · Where your information is kept and who sees it
Your information is stored securely with our technology provider (Amazon Web Services). Only people who need to run the service can access it. We never sell your information, and we never use it for advertising. For full details, see our Privacy Policy.
Your choices and rights
- You can say no to any part of this and still be treated with respect.
- You can withdraw your consent at any time.
- You can ask us to delete everything we have about you.
- You can ask to see or get a copy of your information, or to fix anything that's wrong.

What could go wrong (honest risks)
- The companion may misunderstand you or give an unhelpful answer.
- A screening result could be higher or lower than how you truly feel — it is only a rough check.
- No computer system is perfectly secure, though we work hard to protect your data.
- Sharing with a caregiver means that caregiver will see alerts — make sure you trust them.
If you cannot decide for yourself
If you are not able to make this decision right now, someone legally allowed to decide for you — such as a person you named in a health-care power of attorney, or a guardian — may enroll and consent for you, choosing what you would have wanted. [We may ask for proof of their authority and relationship.]
